Clase and Caermawr Community Development and Outreach Worker
Description
Overview:
We are seeking two part-time experienced Outreach Workers to join our team (21 hours per week).
The role involves working closely with the community to provide a comprehensive package of support to individuals and families. The aim is to empower local people to engage and contribute to community life, fostering a stronger sense of community spirit.
The Outreach Workers will help facilitate groups and activities and provide one-on-one support where needed to address barriers preventing individuals from making positive changes.
Responsibilities
Community Engagement-
· Establish strong connections with local communities, community leaders, and grassroots organisations in the Clase and Caermawr area in Swansea.
· Conduct outreach activities to raise awareness about available health and social care services, resources, and support systems.
· Act as a liaison between community members and relevant agencies, ensuring that their voices are heard and their needs are addressed.
Support and Advice-
· Support the Cwtch Training and Community Coordinator in delivering services across our 4 Cwtches.
· Provide practical support and advice to individuals and families to help address issues affecting them.
· Strengthen partnerships with other projects and organisations, referring clients where appropriate.
· Work with individuals to provide the tools needed to help create a more resilient and solution-focused mindset.
· Listen to the wishes of individuals to ensure they have a voice and that support is client-led.
· Carry out an Assessment to gain an understanding of individual needs.
Collaboration and Reporting-
· Strengthen and develop voluntary and community groups to provide better quality services for children and their families.
· Participate in relevant networks and committees focused on addressing inequalities.
· Deliver family support groups and activities during school holidays.
· Work alongside the wider team to deliver services in our Cwtch and in the community.
· Collect data using the Lamplight database system.
· Maintain accurate records of outreach activities.
· Compile regular reports on progress and impact, delivering findings via quarterly reports and at meetings.
Qualifications
Desirable skills-
· Proven experience in a similar role, preferably within the health and social care sector.
· Excellent communication and interpersonal skills.
· Ability to conduct needs assessments and facilitate workshops.
· Experience in data management and reporting.
· Strong organisational and multitasking abilities.
· Commitment to safeguarding and child protection.
